This form is used to provide narrative comments related to the student's performance.

The form may also be used by
employers for sonographer evaluation.

Continuing Education Record

This form allows an employer to provide a documentation record for each sonographer's Continuing Education. This is one way to ensure clinical competence and compliance with CME requirements in the maintenance of credentials. This is an employer use form and will not be applicable for the student in a sonography program unless the program requires the student to attain CME credit as part of the curriculum.
